Health plans & risk-bearing groups
Find the people who may need more than a snapshot.
Health plans and risk-bearing organizations already know a great deal about their members: diagnoses, medications, utilization, and documented risk factors.
What those records may not show is what happens between visits.
MbeleMed helps identify members who may benefit from additional rhythm monitoring, coordinate access, route findings through qualified physician review, and connect confirmed findings to follow-up.

A visit is one moment. Rhythm changes can happen in between.

Better targeting
Better targeting starts with better questions.
Instead of asking, “How many people can we screen?”
Start with:
- Which members are most likely to benefit?
- What finding are we looking for?
- Who will review it?
- What happens when something meaningful is found?
- How will we know whether the pathway worked?
Aligned incentives
The incentive should follow the clinical finding.
MbeleMed is designed so the program can align around physician-confirmed actionable findings rather than screening volume alone.
That creates a reason to improve targeting, access, review, and follow-up together.
Monitoring services, professional interpretation, downstream care, and other partner-specific responsibilities remain separately defined.
What can be measured
Clinical
- physician-confirmed actionable findings;
- time to follow-up;
- evaluation or treatment initiated by the responsible care team;
- unresolved clinical handoffs.
Operational
- eligible members;
- outreach;
- enrollment;
- monitoring completion;
- report completion;
- follow-up completion.
Utilization
- emergency-department visits;
- admissions;
- downstream procedures;
- specialty follow-up;
- other pre-specified measures.
Programs should define their measures before launch. MbeleMed does not promise predetermined savings or utilization reductions.
What can be found
The heart does more than one thing wrong.
Depending on the protocol, programs can look for:
- atrial fibrillation and flutter;
- supraventricular and atrial tachycardias;
- ventricular arrhythmias;
- bradycardia and significant pauses;
- conduction disease, including AV block;
- clinically meaningful ectopy;
- other supported rhythm abnormalities.
FAQ
What does “actionable” mean?
A physician-confirmed finding that meets the agreed program criteria for follow-up, further evaluation, treatment consideration, or escalation.
Does MbeleMed make the monitor?
No. MbeleMed does not manufacture or own monitoring devices.
Who reviews the findings?
Qualified physicians under the applicable monitoring and clinical pathway.
Does MbeleMed diagnose or treat people?
No. MbeleMed coordinates the workflow. Qualified clinicians retain clinical authority.
Does MbeleMed submit medical claims?
No.
Does every program work the same way?
No. The population, monitoring service, clinical pathway, responsibilities, and evidence question are defined for the actual program.
What happens if the monitoring is negative?
A negative result can still be useful information. Monitoring, professional interpretation, and other program costs remain separately attributable to the applicable parties.
